Three Options for submitting New Patient Forms
1. New Patients may print forms to fill out prior to their appointment

Fill them out and get them to us at your convenience
You may:
Drop them off anytime before your appointment
fax them to us at 770 949-0707
Bring them to your appointment
Getting these forms to us at least 72 hours prior to your appointment allows us to verify and process your insurance benefits for you so that you may only be required to pay your deductible and your co-pay on the date of service if your insurance has been approved.
Otherwise, we do ask that payment be paid in full on the date of service and we will be happy to submit your insurance for reimbursement.
2. New Patients may fill out our secure on-line forms linked at the top of this page.
This will also give us the chance to verify and process your insurance information for you prior to your appointment. Please fill these out at least 72 hours before your appointment when using this option.
3. New patients may fill out forms at our office 15 minutes prior to their dental appointment.
We ask that you please pay for services in full at your first appointment if choosing this option.
